Job Description

MetroWest Center for Independent Living (MWCIL) is a 501(c)(3) non-profit organization that works to empower individuals with disabilities to become active members of the communities in which they reside. Our service area includes 26 cities and towns in the MetroWest area of the state.

We are currently seeking a full-time Long Term Services and Supports (LTSS) Coordinator to provide the five core services of Independent Living to individuals with disabilities who are either living or transitioning towards living in the community, as well as to provide Long Term Services and Supports (LTSS) to consumers in the community with specialized health insurance.

Responsibilities

  • Provision of the 5 core services of independent living: information and referral, IL skills training, peer counseling, advocacy, and transition.
  • Provision of LTSS services to consumers in the community who have specialized health insurance benefits.
  • Assessment of an individual’s ability and service needs to live independently.
  • Act as an independent facilitator and liaison between Enrollees, all of the contracted One Care Organizations, and service providers.
  • Advocate for and with Enrollees regarding his/her LTSS needs.
  • Educate Enrollees and his/her Interdisciplinary Care Team on LTSS.
  • Arrange and coordinate the provision of community in home supports and services.
  • Develop and periodically review an Independent Living Service Plan with assigned consumers.
  • Document work by entering data accurately and in real-time into the data collection systems used by LTSS contracted agencies and MWCIL’s own web-based data collection system.

Qualifications

  • Bachelor’s Degree in related field such as social work or human services
  • 1 year relevant work experience.
  • Excellent verbal, written, and interpersonal skills, including the ability to interact with diverse populations.
  • Knowledge of community resources and local service system for persons with disabilities and how to arrange for services.
  • Personal knowledge of disability issues preferred.
  • Working knowledge of Windows and Microsoft Office applications.
  • Fluency in Spanish, both verbal and written, preferred
  • Ability to make home visits where physical access may be problematic.
  • Travel within service area and beyond.
  • Must have own reliable transportation and current MA driver’s license.
