One Care is a Self-Directed Health Care model for individuals enrolled in MassHealth and Medicare. MWCIL’s Long Term Services (LTS) Coordinators work with individuals to establish the supports necessary to improve health. LTS Coordinators act as both an independent advocate for the member and as a resource to connect them with long-term service options they might be eligible for in the community.
